TCOYD® Diabetes Self-Management Education and Support (DSMES) Program
TCOYD® Diabetes Self-Management Education and Support (DSMES) Program
Diabetes can be overwhelming and managing diabetes can be challenging—especially for the person living with diabetes. After all, 99% of the important daily management decisions are made by YOU.
Learning to be a self-manager of your diabetes is lifesaving. Diabetes Self Management Education and Support (DSMES) provides the foundational knowledge and skill to make the day-to-day decisions that will impact blood glucose control, heart and kidney health, and body weight. DSMES teaches you how to decrease anxiety and distress which can prevent hospitalizations and emergency department visits.
Whether you have type 1 or type 2 diabetes, or care for someone with diabetes—type 3s as we call them—TCOYD has developed an education program for you. At TCOYD you will find different ways to learn and different topics to help you learn. You can learn what you want, when you want, from your own home.
TCOYD has developed a DSMES program to help you stay active in your diabetes management. If you are interested in exploring our DSMES program you can answer a few questions about your management, select your topics of interest, and participate in the online learning. After you complete an activity, you will receive a certificate of completion that you can share with your healthcare provider.
Please note this program is still in early development and we welcome your feedback.
