I’ve been a Palliative care pharmacist my entire career. I optimize my many years of experience to ensure my patients are comfortable and symptom free at the end of life. This is done through advanced pain management experience using traditional opioids and non-traditional medications such as Methadone and Ketamine. I pride myself on DEPRESCRIBING medication. In my chart reviews, more than 50% of prescribed medications were prescribed to treat side effects of other non-essential medications. We also use alternative administration routes such as rectal…..where most drugs actually gain better bioavailability. In collaboration with the interdisciplinary team, I assess the needs of each patient, including family dynamics and spirituality, to select the best method of treatment. This team may include a spiritual counselor, nurses, physicians, caregivers, and volunteers.
